To register for Online Services please attend Oldfield Surgery site or St James Surgery site with Photo ID and proof of address.

You are now able to do the following, Online.

  • Order repeat medication
  • View your Summary Care Record access
  • View Detailed Coded Record Access (includes viewing test results)
  • Request Full Clinical Record access (this includes Detailed Coded Record access)

IT/Electronic patient records – statement of intent for Heart of Bath Medical Partnership

Contractual requirements came into force from 1st April 2015 requiring that GP practices should make available a statement of intent in relation to the following IT developments:

  • Referral Management
  • Electronic Appointment Booking
  • On line Booking of repeat prescriptions
  • Summary Care Record
  • GP2GP transfers
  • Patient Access to the detailed information from the medical record.
  • Full Clinical Record Access

Heart of Bath Medical Partnership now offers our patients all of the requirements under the GP contract relating to IT developments and are working closely with our Clinical System Suppliers to allow patients to access the detailed information from their medical record.

Patients can now access their Full Clinical record information online, to do this you will need an online account. (Please note Full Clinical Record access includes Detailed Coded Record access).

* Printing a paper copy of your medical records takes approximately 10 weeks to complete.
Full Clinical Record Access – Online Access
Requests will need to go to your registered GP for authorisation, this is currently taking between 6 to 8 weeks to process, depending on GP admin workload.

NHS App only When setting up the NHS app only you will get to view basic information, this includes, booking appointments, ordering repeat medication, viewing your Covid Vaccinations.

To see more detailed access (Detailed Coded and Full Clinical) you will need to contact the surgery directly to get this activated.

It is advisable to get a Heart of Bath live online account as an alternative to viewing your full record on your NHS app. Heart of Bath online access gives a more detailed view.


Proxy Online access  – via an individuals own online account. Processing for access up to Detailed Coded Record access is currently taking 14 days.   Full Clinical Record access will take 6-8 weeks to process, depending on  GP admin workload.

Child up to the age of 11  Appointment booking, Repeat Medication ordering, Summary Care Record access and Detailed Coded access, which includes test results and vaccination history is given if requested by a parent/guardian.  Full Clinical record access requests from parent/guardian will need to go to a child’s registered GP for authorisation, this is currently taking between 6 – 8 weeks to process, depending on the GP’s admin workload. Access will automatically end on the child’s 11th birthday. Re apply via the surgery for access between 11- 16 years.

Child between the ages of 11 to 16 years – Appointment booking only, is given to parent/guardian due to the sensitivity of the child’s age. Repeat Medication ordering requests from parent/guardian is only given where there is written, signed consent from the child (consent forms can be obtained from our surgery receptionists). Detailed Coded Record access, which includes test results and vaccination history, and or Full Clinical Record access requests from parent/guardian need to be reviewed and discussed via a telephone/face to face appointment with the child’s registered GP to gain consent from the child. Access will automatically end on the child’s 16th Birthday. The follow-on admin process for this access is currently taking between 6 – 8 weeks to process, depending on the GP’s admin workload.

Adult proxy access – Access request from parent/guardian/relatives is only given with written, signed consent from the adult over the age of 16. If the patient is over 16 and incapacitated or not competent, requests will need to be discussed and reviewed  via a telephone/face to face appointment with the adult over the age of 16’s registered GP to gain consent from that adult.

NHS App

Without requesting online access from your GP surgery you can now download the NHS app, which gives you access to order repeat medication, view your summary care record and view your covid vaccination history only. For further detailed access via the NHS app you will need to apply directly for online access as above through the surgery.
